Sara Roman serves as the Administrator of Choice Hospice, overseeing the organization’s daily operations and leadership. Since joining Choice Hospice in 2019 as the Office Manager, she has grown with the organization through her dedication and commitment to excellence. Sara is passionate about supporting patients, families, and staff while ensuring compassionate, high-quality hospice care.

Knapp
Anna Knapp serves as a Hospice Liaison with Choice Hospice, supporting patients and families as they navigate the end-of-life journey with compassion, education, and guidance. She has been with the Choice Hospice team since 2021 and brings more than 14 years of hospice experience, along with a background in business, education, and direct patient care. Anna is dedicated to helping families understand their options.

Diane Havaich-Onken serves as the Assistant Director of Nursing at Choice Hospice, supporting daily clinical operations and the nursing team. She is committed to fostering teamwork, maintaining high standards of care, and ensuring patients and families receive compassionate support throughout their hospice journey. Diane has been a nurse with Choice Hospice since 2018, bringing extensive experience and dedication to the organization’s mission.

Danielle Smith serves as the Director of Nursing at Choice
Hospice, leading the clinical team in delivering compassionate,
patient-centered hospice care. She oversees nursing services, supports clinical
excellence, and guides her team in providing individualized care that promotes
comfort and quality of life. Danielle has been a nurse with Choice Hospice
since 2021, bringing dedication and experience to the organization’s mission of
quality end-of-life care.
Dr. Frank Yono
Dr. Frank Yono has served as the Medical Director of Choice Hospice since its founding in 2015. Inspired by a desire to improve the hospice experience for his own patients, Dr. Yono partnered with the founders of Choice Hospice to create an organization where patients and their loved ones have a true choice in their end-of-life care. His vision was and continues to be to provide compassionate, individualized care that honors each patient’s unique needs, goals, and wishes.
As Medical Director, Dr. Yono provides physician leadership and medical oversight with a strong focus on comfort, dignity, and quality of life. Working alongside the interdisciplinary hospice team, he helps develop personalized care plans, manage symptoms, and ensure every patient and family receives the support, respect, and guidance they deserve throughout their hospice journey.
